Medicare generally covers most outpatient mental health services, including marriage counseling. However, various out-of-pocket costs may be involved with this care.
Medicare Part B covers outpatient mental health services, including marriage and family counseling.
Part B provides coverage for various mental health services from mental health professionals, including:
- clinical psychologists
- physician assistants
- nurse practitioners
- psychiatrists and other doctors
- mental health counselors
- marriage and family counselors
Medicare Advantage (Part C) plans will provide the same coverage as Part B for marriage counseling.

Individuals with Original Medicare (parts A and B) must pay the Part B monthly premium, which is $185 or higher, depending on income.
Once a person has reached the Part B deductible of $257, they are only responsible for 20% of the Medicare-approved costs for mental health services, including marriage counseling.
Individuals with Medicare Advantage are still responsible for the Part B premium. They must also pay their plan’s monthly premium and meet any deductibles included with their plan.
